Smith served in the army in Germany before studying medicine at the University of Manchester. After qualifying, Smith was ill for some years, but was house surgeon at Ashton-under-Lyne. He then decided to specialise in ENT and was clinical assistant at the St John's Hosptial for Diseases of the Ear, Manchester, house surgeon to the ENT department at Oldham Royal Infirmary, RSO at Manchester Ear Hospital then clinical assistant at Edinburgh Royal Infirmary. After being made a fellow of the Royal College of Surgeons of Edinburgh, Smith returned to Manchester. He was clinical assistant to the Aural department of MRI, and held consultant and honorary positions at the Hospital for Diseases of the Chest, Ear, Nose and Throat, St John's Hospital, Crumpsall Hospital, Ancoats Hospital and the Manchester Royal Eye Hospital.
