Smith, who later became known as 'Fat' Smith, was born in Lancaster on 13 April 1864 and gained his medical education at Edinburgh University where he was demonstrator in anatomy. He then came to Manchester as demonstrator in anatomy before being appointed resident surgical officer to MRI in 1891. Smith volunteered for the Boer War in 1900, then returned to Manchester in 1901 and was appointed honorary surgeon at MRI and visiting surgeon to Withington Hospital. In 1911, Smith was elected professor of systematic surgery at the University of Manchester, having been lecturer in operative surgery for the previous six years. In 1914, along with Westmacott was responsible for the mobilisation of the Second Western General Hospital. Smith was president of Manchester Medical Society in 1919, and of the Manchester Pathological Society and the Royal Medical Society of Edinburgh. Smith retired in 1922 and moved to Ingleton, Yorkshire, where he died on 13 April 1926.
